What is a preauricular sinus?
A preauricular sinus is a benign congenital anomaly presenting as a small pit anterior to the external ear, typically arising during the sixth week of gestation. Without proper hygiene and clinical care, it may predispose patients to complications such as localized infection, abscess formation, and secondary cysts.

Classification of Preauricular Sinus
The condition is categorized by the anatomical location of the pit—preauricular, supra-helical, tragal, or lobular. It is further clinically staged by the severity of infection: uninfected, acute infection, chronic infection, or complicated.

Etiology of Preauricular Sinus
This is a congenital malformation caused by a defect or incomplete fusion of 6 auricular folds during embryonic development around the 6th week of pregnancy.

Clinical Manifestations of Preauricular Sinus
The symptoms of the disease include the appearance of a small hole in front of the ear, which may discharge white or yellow fluid with a foul odor. When infected, the disease can cause symptoms such as swelling, redness, pain, itching, fever, pus discharge, and headaches.

Clinical Treatment Protocol

• Local management: For fistulas in the acute abscess phase: Incision and drainage of pus

• Surgical management: For stable, quiescent fistulas without active inflammation: Complete surgical excision of the congenital preauricular fistula tract

Preventive Care for Preauricular Sinus
Preventive management involves comprehensive neonatal screening, maintaining meticulous local hygiene, preventing the entry of particulate matter, water, or irritants into the orifice, and seeking prompt medical evaluation at the earliest sign of infection.
